Community Health Assessment

A core function of public health is assessment. The purpose of the community health assessment is to learn about the community: the health of the population, contributing factors to higher health risks or poorer health outcomes of identified populations and community resources available to improve the health status.

A community health assessment is a collaborative process of collecting and analyzing data and information for use in educating and mobilizing communities, developing priorities, garnering resources or using resources in different ways, adopting or revising policies and planning actions to improve the population’s health.

**July 2024** If you live or work in Galion/Crawford County, please take a few minutes to take this Community Health Assessment Survey so that we may better understand the health needs and concerns in the community.

***MAY 2023 UPDATE FOR GALION RESIDENTS*** - we would love your feedback on the results of a health report we completed about Galion, before we officially release it. This health report is an addendum to the 2022 Crawford County Community Health Assessment (if you'd like to review that, the link for that is below). This Galion-specific addendum was necessary as we work towards becoming a nationally accredited health department. Please view it by clicking this link: Galion CHA addendum. To submit your feedback on the Galion CHA addendum after reviewing it, please send an email to [email protected].
