Immunizations Offering immunization appointments for infants, children, teens and adults

Immunization Appointment Hours*

2nd Wednesday of the Month: 9:00 am – 11:00 am & 1:00 pm – 3:00pm
3rd Wednesday of the Month: 3:00 pm – 6:00 pm
4th Wednesday of the Month: 9:00 am – 11:00 am & 1:00 pm – 3:00 pm
*MUST call to schedule an appointment, (419) 468-1075 and have insurance information ready.

To help speed your appointment, you can complete the appropriate screening form (Child Screening Form or Adult Screening Form) and bring it with you to your appointment.  If you have not completed a HIPAA form within the last calendar year or if your information has changed it will need to be completed.  Also if you have never completed a Disclosure, you may compete it as well.  (If a minor is brought in for care by someone other than his/her parent/guardian, the person who brings the child must be 18 years of age or older and must have written parental/guardian consent to treat the minor).

Infant and Child Immunizations

Pediatric patients with insurance MUST provide ANY and ALL insurance cards/information at the time of service.
Many private insurance and medical cards are accepted for childhood immunizations. Galion City Health Department is out-of-network with Aetna.

Fees for pediatric patients with NO insurance or who are underinsured (per VFC guidelines) and paying at the time of service are as follows.
1st immunization: $15.00 (not including flu shots, check for current pricing)
Each additional immunization per visit is $10.00 (not including flu shots, check for current pricing)

No VFC eligible child 18 or younger will be denied required immunizations due to the inability to pay.

Adult Immunizations

The following immunizations are available for adults:
 
Hepatitis A Series (Series of 2)
Hepatitis B Series (Series of 3)
Gardasil 9 Series (Series of 3) (Females & Males up to age 45) 
Polio
Tetanus and Diphtheria (Td) - Booster recommended every 10 years
Tetanus, Diphtheria, and Pertussis (Tdap)
Meningococcal (Meningitis)
MMR
Flu - call for current pricing
