Birth Certificates

Galion City Health Department issues certified copies of birth records for births in Ohio from December 1908 to present. For births prior to December 1908, see the Vital Statistics main page.

Each certified copy is $25.00. Uncertified copies of birth certificates are not issued.

Walk-In

Complete and print an Application for Certificates and bring it to Galion City Health Department (applications can be obtained in office also). Only cash, Ohio checks, or money orders are accepted. Office hours for this service are Monday – Friday, 8 am – 4 pm.

Online

The Galion City Health Department has partnered with Permitium to facilitate online ordering of vital statistics.  To order online visit  https://galionoh.permitium.com/rod.  This service has an additional $5 convenience fee and a credit card processing fee.

Mail Order

Complete and print an Application for Certificates and mail it, along with a self-addressed, stamped envelope and an OHIO check or money order for $25.00 times the number of copies to:

Galion City Health Department, 113 Harding Way East, Galion, OH 44833. Mail orders are typically processed and sent out within one to two business days.

Correcting a Birth Certificate

Correcting a birth certificate (most often a misspelling), which requires an affidavit, can be initiated at our office. Walk-in service is offered for these corrections.

Once you have verified the information on the birth certificate and the information to be changed with our Registrar, the affidavit is then sent to the state so that the birth certificate can be updated.

Adding a Father to a Birth Certificate

Single, unmarried mothers wishing to add a father to a birth certificate can initiate the process at our office. Walk-in service is offered for this addition and requires that both parents be present.

Our Registrar will complete an Acknowledgement of Paternity Affidavit, obtain both signatures, and then send the affidavit to the Central Paternity Registry.

The form can also be taken home so the parents can read over the affidavit. However, signatures must be notarized, so only sign the form while in the presence of a notary. The process can take up to 2–3 months for the name to be added to the certificate.
