A core function of public health is assessment. The purpose of the community health assessment is to learn about the community: the health of the population, contributing factors to higher health risks or poorer health outcomes of identified populations and community resources available to improve the health status.

A community health assessment is a collaborative process of collecting and analyzing data and information for use in educating and mobilizing communities, developing priorities, garnering resources or using resources in different ways, adopting or revising policies and planning actions to improve the population’s health.

From the beginning, community leaders were actively engaged in the planning process and helped define the content, scope, and sequence of the study. Data was collected through health surveys mailed to a random sample of Crawford County adult residents; as well as an additional sampling of Galion City adult residents. A random sample of students were selected and surveyed in Crawford County schools.  The goal was to identify key strengths, weaknesses, and health concerns for residents of Crawford County and Galion City.

The members of the Crawford County Health Partners are pleased to present the 2016 Crawford County & Galion City Community Health Status Assessment. 

This comprehensive Community Health Assessment is the result of a strong commitment by dedicated community partners working together to improve the health and quality of life of Crawford County/Galion City residents. The health assessment gives us a snapshot of our community as well as a comparison to the state of Ohio and our nation.  The 2016 Crawford County & Galion City Community Health Status Assessment was made possible through funding from the Galion City Health Department, Crawford County Public Health, Crawford-Marion County ADAMH Board, Community Foundation for Crawford County, Crawford County Education & Economic Development Partnership, City of Bucyrus, and Avita Health System.
